Community Health Educators
REACH Community Health Educators will conduct two workshops per week over twelve months in targeted census tracts in Danville City and Pittsylvania County. Workshops will be based on specific population health topics, addressing areas of high-need and chronic health issues. Members will serve approximately 30 hours/week
Member Duties : • Obtain certification from evidence-based training programs to implement workshops. • Conduct, support, and create workshops to address community health needs. • Support outreach events. • Provide assistance and referrals in helping individuals address social determinants of health to include transportation, housing, food insecurity, economic distress and childcare. • Refer individuals to REACH Community Health Workers • Keep records and report to Coordinator on a regular basis. • Document progress of participant's goals and administer pre/post assessments. • Collaborate with staff, fellow AmeriCorps members, and partner organizations to develop and support service projects in the primary areas of healthy futures. • Recruit volunteers to support identified service opportunities. • Attend all mandatory meetings. • Attend all planned training sessions for REACH members. • Complete and submit accurate and timely documentation • Participate in any service projects
