Assess and provide intervention for individuals with a wide range of chronic medical conditions, and behavioral health issues affecting medical conditions to include assessment and disposition of beneficiaries with acute and chronic risk. Consult with primary care providers to select, approve, and document treatment priorities. Educate clinic staff on the principles of primary care behavioral health via trainings and briefings. Conduct outpatient group classes, stress management, relaxation training, assertive/social skills training, diabetes management, chronic pain, and brief related interventions. Assess acute mental and behavioral medicine conditions in beneficiaries. Provide individual and group treatment, using various evidence-based modalities to include problem focused interventions, cognitive-behavioral therapy, and psychoeducational classes.
