Positions Available:

  • PRN, PerDiem
  • Outpatient setting
  • Monday - Friday
  • Clinic hours of 0700-1800
  • Never any on-call!
  • Possess a minimum of 1 year experience as an LPN working in an outpatient acute care (hospital based) or ambulatory care setting within the last two years.
  • Pay $35/hour plus additional $4.23/hour = $29.23

Key Responsibilities
  • Observes and records pertinent vital signs, determines and orders laboratory procedures on patients that may be ordered by the provider to diagnose and treat the patient.
  • Utilizes automated programs and information technology tools to facilitate the patient experience and screen Medical Home Port patients, needing preventative services.
  • Tracks HEDIS measures, laboratory/radiology results and notify patients.
  • Check patient identification, Healthcare Provider's orders, and transfer information as applicable.
  • Administer medications, orally and injectable, monitor and maintain intravenous fluid.
  • Assists with clinic check-out and follow-up visits by reviewing post-visit needs with the patient.
  • Ensure scheduling of follow-up appointments or specialty care referrals. Assist patients by directing or escorting to other clinical services.
Skills, Knowledge and Expertise
  • Possess an active and unrestricted Licensed Practical / Licensed Vocational Nurse license.
  • Be a graduate from an LPN / LVN program.
  • Have 1 year of full-time experience as an LPN/LVN in a hospital based or outpatient primary care or ambulatory care setting.
  • Basic Life Support certification from an American Heart Association program.
