Description:

The City of Virginia Beach Department of Human Services is seeking a BH/DS Clinician II - House Manager to work in one of our Group Homes in Developmental Services. As a Team Coordinator, this position is primarily responsible for providing staff supervision and leadership and ensuring quality of care for all participants. The Team Coordinator is also responsible for coordinating services and support via a person-centered plan that addresses complex behaviors, medical needs and crisis issues. In addition, this position is tasked with enforcing a multitude of state regulations and City policy and procedure. The ideal candidate will have experience working with adults who have Intellectual and Developmental Disabilities in a residential setting.

Duties:

  • Develop, implement, and monitor individual service plans (ISPs) for each participant, ensuring outcomes and essential services align with their needs and abilities.
  • Conduct regular assessments of participants' physical, emotional, and social needs to identify and address necessary supports.
  • Coordinate and facilitate access to healthcare, therapy, vocational training, and other services required by participants.
  • Maintain accurate and timely documentation of case files, including assessments, progress notes, and incident reports, in compliance with regulatory requirements.
  • Track and report participants' progress toward achieving goals outlined in their ISPs.
  • Work closely with direct support staff, healthcare providers, and therapists to ensure the overall health, safety and wellbeing for participants.
  • Provide guidance and support to staff regarding participants' needs, care plans, and behavior management strategies and to ensure accurate and consistent data entry into organizational systems.
  • Communicate regularly with participants, families, or guardians to provide updates and address concerns.
  • Assist participants in building and maintaining social connections within their communities.
  • Assist in the development and implementation of behavior intervention plans in collaboration with specialists.
  • Train and guide staff on appropriate responses to behavioral and medical challenges and emergencies.
  • Review and provide feedback on documentation and hourly billing of Direct Support Professional IIs (DSP II) and Behavior Specialist Is (B-Spec I) for accuracy, adequate details, and compliance while immediately reporting any deficiencies to the supervisor.
  • Sign and verify quarterly reviews in the electronic health record by the quarterly due date and provide accurate details related to each activity and essential support based on how the person was supported, and how they responded to the supports.
  • Prepare or review the medication administration record prior to implementation to ensure all entries match the physician's orders and that all necessary medications are on-site, available, and appropriately labeled.
  • Check or verify medications delivered for accuracy and report inaccuracies immediately to the pharmacy and /or the Case Coordinator, as well as completing monthly controlled medication counts for participants who are administered narcotic medications.
  • Serve as an after-hours standby supervisor on a rotating basis.
A $4,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.
  • $1,000 will be awarded in the first paycheck.
  • $1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of "Meets Expectations".
  • $1,750 will be awarded upon completion of the employee's one (1) year probationary period and a performance evaluation with an overall rating of "Meets Expectations".
