The Community Health Worker (CHW) serves as a trusted liaison between health and Bon Secours Maryview Medical Center and the community. This role involves providing culturally appropriate health education, outreach, support, and advocacy to improve access to services and promote healthy behaviors in underserved populations. Assists with marketing programs and services, as well as data collection. Build and maintain trusting relationships with community members. Conduct outreach to identify individuals and families who may benefit from health and social services. Provide basic health education and information on available resources.
